MONITORING FORMS

Standard Monitoring Forms. General use monitoring forms for all types of accounts. Includes New Subscriber Data Form, Subscriber Changes Form, and Cancel Form.

Medical Alert Monitoring Forms. Specific to Medical Alert dealers: Includes a New Client Data Form and an optional Billing Authorization form.

Customer Monitoring Agreements. General-use forms between your company and your customer. They can be customized with your company name and logo upon request. We offer two variations, with and without term lengths:

Customer Insurance Certificate. Most Insurance companies give a 10-20% insurance discount to customers with alarm systems. This form can be completed upon installation and supplied to the customer to give to their insurance company.

New Web Access Guide. Our new web access system was deployed in April, 2008. Those of you that have been using the old system will be pleased with the enhanced functionality. Please note that you will need to set up a new security account the first time that you use the new web access.

2400A Software and Instructions. (updated 9-14-06) 3.6meg zip-file that contains the Windows software to program and remotely access the 2400a medical alert system. Also includes our 2-page setup guide and Linear's reference manual. NOTE: You will need a modem to access and program these systems. Please review the 2-page setup guide for details.

Medical Alert Direct Shipping Forms. Includes equipment order form and customer shipping authorization form. If you use our Direct Shipping program and order 5 or more kits, we will handle everything for you, from setting up the medical alert systems, shipping them in your name, and assisting your customers in setting up their systems.
